The Beacon Institute of the LifeSpan Network presents its annual Leadership Summit for Maryland home care, nursing home, CCRC and assisted living executives. This year's topic is "The Role of Post-Acute Services in Preventing Hospital Readmissions."
All Maryland hospitals will be represented, and the conference will bring together key leaders in the acute and post-acute sectors to learn about best practices in working together to prevent re-admissions; share their own experiences and challenges; and network and forge alliances with their peers in the acute and post-acute sectors with the goal of benefiting patients/residents and providers alike.
